Elmora Youth League is a nonprofit organization based in Elizabeth, New Jersey, The league organizes recreation baseball, softball and basketball for the youth of the Elmora/Elizabeth area. The league is staffed by volunteer parents. EYL forms travel baseball and softball teams that participate in local summer tournaments. Players are selected from registered participants in the Spring season.

	This form is reviewed by our safety team. All issues are reviewed with coaches and the coaches carry these forms with them to all practices and games.
